Cosmetic surgery may seem like choosing from an a la carte menu: a little work on the nose here, eyelid lifts there, and maybe a facelift for dessert. Yet some procedures seem to naturally complement each other, just like wine can complement a main course.

For most of us, work is stressful. But for Avani Nayak, her job actually makes her more relaxed. “We have a beautiful, relaxing atmosphere; and it always smells wonderful in here,” she says of her Avani Day Spa in Frontenac. “And I love seeing the looks on people’s faces when they leave: They’re smiling and have more self-confidence, and they feel relaxed and invigorated.” We asked her about the rough and dirty business of getting the best care for your skin.

There are nose jobs, and then there are nose jobs. Surgical revision of the nose may be purely cosmetic (rhinoplasty) or necessary to correct a functional problem caused by a genetic defect or trauma. And in some cases, the problem is both cosmetic and functional.
You have an hour, and then it’s back to the office. What can you do during that time to improve your appearance without needing to hide afterward while you heal from the procedure? Surprisingly, quite a lot.
Sometimes when you’ve tried everything else, it’s time to take the leap. And when the issue is wrinkles, that final leap may be into the operating room. Surgical procedures to correct facial lines and wrinkles are no longer restricted to a complete facelift, with options based on individual needs and circumstances. In some cases, surgery is accompanied by less-invasive procedures that provide a final touch to create the optimal result.

We salute longtime John Burroughs School head football coach TODD SMALL, who is retiring at the end of the school year. The announcement comes at the conclusion of a banner season for JBS that included a record 14 wins. Small, who also teaches history at the school, has coached high school and college students for 40 years, 23 of them at Burroughs. Succeeding him is former NFL quarterback GUS FREROTTE, who joined the Burroughs coaching staff as the quarterback coach in 2009. Drafted by the Washington Redskins in 1994, Frerotte also played for the Detroit Lions, Denver Broncos, Cincinnati Bengals, Miami Dolphins, Minnesota Viking and St. Louis Rams.

Yet another disappointing aspect of aging is hair loss. Back in the 1940s, a dermatologist discovered that certain hair follicles are programmed to last a lifetime, while others have an expiration date. He also found that if you transplant those lifetime follicles to another location, they will retain that programming, explains Dr. Bruce White of St. Louis Cosmetic Surgery. The lifetime follicles tend to be lower on the head, around to the back, explaining the common male-pattern baldness.

Most patients thinking about facial rejuvenation have one common request: I want it to look natural.
Lasers are buzzwords in the aesthetic industry. While they have their place in skin rejuvenation, it pays to choose wisely, according to the medical experts. “Lasers are fancy flashlights,” says Dr. Michael Nayak of Nayak Plastic Surgery. “We shine them on skin and they can effectively treat many issues. Skin is complicated: It has many structures living in it. Lasers can effectively kill the parts we don’t like.” He says different lasers have different functions and can be either ablative (causing raw skin), or non-ablative, which require much less downtime, but are somewhat limited in what they can achieve.
Whether your friends call you elephant ears, or your 2-year-old just ripped an earring out of your lobe, you may need otoplasty, the medical term for surgical repair, restoration or alteration of the auricle (external portion) of the ear. Plastic surgeon Dr. Judith Gurley does a number of otoplasty procedures. “I do ear pinning to put prominent ears into a more aesthetic position, closer to the side of the head,” she says. “It’s very valuable to reduce embarrassment and teasing; it really improves self-confidence and allows girls to wear their hair back and boys to wear their hair short.” Gurley says ear pinning should wait until the cartilage has matured enough to hold stitches, generally by age 7.
Do friends sometimes think you’re angry, or tell you to smile? There is a cure for that furrowed brow that some of us develop with age: it’s called a brow lift. “Many people, when they hear ‘brow lift,’ think of an overdone, startled look, so I call it ‘brow shaping,’ ” says Dr. Michael Nayak of Nayak Plastic Surgery. Brow lifts are done to raise the brow, not to get rid of crows’ feet or the creases between the eyebrows, he explains. It can help those lines, because during the procedure the muscles that cause them are removed, but they’re not the main target. If, when you raise your brow manually, excess skin above the eyes disappears, only a brow lift is needed.
One woman underwent rhinoplasty at Nayak Plastic Surgery & Skin Enhancement Center. According to Dr. L. Mike Nayak, the procedure can be performed without general anesthesia, and patients can be ready to go back to work or school within a week.
For most of us, the first sign of facial aging occurs around the eyes. (Droopy jowls come later, thank goodness!) So we seek out a plastic surgeon for an eye lift, or blepharoplasty, and are surprised when we’re told that the real problem is a sagging forehead (brow). Dr. Mark Checcone of Facial Plastic Surgery Center at Washington University says his patients complain of excess eyelid skin or that they are frequently told they look tired or angry. “I take the time to educate them about how lifting the brow can open up the eye area, improve the contour of the brow, and reduce excess upper eyelid skin. When I manually lift the brow, they can see the effect.”
